Emergency planning

If you couldn't provide care, would someone know what to do?

An emergency plan records the important information someone else may need if illness, an accident or another unexpected situation means you can't carry out your caring role.

Your plan

We'll help you build it step by step

You don't need to have everything worked out before you start.

  • Important information about the person you care for
  • People and services to contact
  • Medication and health information
  • Daily routines and practical needs
  • What should happen if you can't provide care
  • A finished plan you can print or save as a PDF
Before you start

Have the key details to hand

It helps to have emergency contacts, GP details, medication and health information, and the person's usual care arrangements nearby. Your answers are not saved online, so complete the plan in one session and print it or save it as a PDF before leaving the page.

Digital tool

Create your emergency plan

Work through the six short sections below. Add as much information as is useful to you. You can then review your plan and print it or save it as a PDF.

Your information stays with you

This tool works in your browser. Your answers are not submitted to Liverpool Carers Centre and are not saved online. Complete your plan in one session, then print it or save it as a PDF before leaving the page.

About the person you care for Step 1 of 6
Step 1 of 6

About you and the person you care for

These are the personal details included in the Liverpool Carers Centre paper emergency plan.

Step 2 of 6

Emergency contacts and GP

Add the two emergency contacts and GP details asked for on the paper plan.

If you do not have an emergency contact, the paper plan advises using Liverpool City Council Adult Social Care: 0151 459 2606.
Step 3 of 6

Conditions, medication and nutrition

Record the medical information somebody may need if they have to step into your caring role.

Include medication names and doses where useful.
Include what is needed in the morning, at lunch and in the evening, and note if this changes on particular days.
For example: NGT, soft foods, thickened fluids, dietary needs or support with eating and drinking.
Step 4 of 6

Care agency, day centre and regular support

If other services are involved, record who they are, when they attend and what they do.

For example: personal care, meals, medication, tea call, back to bed or comfort call. Include the relevant days of the week.
Step 5 of 6

Need to know

Give a short overview of the person's needs, routine, likes, dislikes, challenges and any equipment used in their care.

Think about routines, communication, personal care, mobility or equipment, what they enjoy, what may upset them and what helps them feel safe.
Step 6 of 6

Other important information

Use this final section for anything important that has not already been covered.

You can use this for more detail about medication, care arrangements or anything else someone may need in an emergency.

Before we create your plan

We'll flag a few particularly useful pieces of information if they are blank. You can still create your plan even if you do not know everything yet.

Carer emergency card

Keep a reminder with you

If something happened to you while you were away from the person you care for, a carer emergency card can let someone know that another person may be relying on you.

The card shown here is only a sample. Emergency cards are issued through Liverpool Carers Centre and can't be created or printed from this website.
